Tallwoods Care Center provides the services of a certified social worker to meet the
psychosocial and emotional needs of our residents. Whether setting up services for
discharge following a rehabilitative stay or helping someone adjust to new living
arrangements, the role of the social worker is to help and guide the resident and family.
The role of the social worker is quite diversified. The social worker will plan, with
assistance and guidance from the family, comprehensive discharge services to facilitate the
resident's reentry into the community. They will also guide the resident and family in
obtaining supplemental services. In addition, they can advise residents and families
regarding insurance, Medicare and Medicaid issues.
Should a resident require hospice care while in the facility or in the home upon discharge,
the social worker will guide the family and help secure these services on behalf of the
resident. |
